Thyroid gland is located at the front of the neck and mainly produces the hormone thyroxine, also known as T4. The hormone affects how quickly the body uses energy, how heat is produced and how the brain, muscles, intestines and heart work. When levels become too low, the pace of many organ systems is slowed down at the same time.
That is why the fatigue of hypothyroidism often feels different from regular lack of sleep. Many describe a toughness that does not go away with rest, as if the whole body responds more slowly than before. At the same time, the symptoms can be diffuse, especially in the beginning, which makes the condition easily confused with stress, depression, menopause or iron deficiency.
Why hypothyroidism makes you tired
Fatigue in hypothyroidism occurs when the body's cells receive too little stimulation from thyroid hormone. This reduces energy metabolism in the tissues, which affects both physical strength and mental acuity. The brain can feel slower, the muscles heavier and recovery is poorer.
It is often noticeable in completely ordinary situations. A walk that previously felt easy can now feel unusually heavy. Tasks that require focus take longer, and small interruptions can make it difficult to get back into the mindset.
Hypothyroidism also affects the body's temperature regulation. Many people get cold easily, especially in the hands and feet, and this can reinforce the feeling that the body is running on reserve. If the pulse also becomes lower and the intestines work more slowly, there is a general feeling that the system is in slow motion.
Common symptoms of hypothyroidism that are often missed
Hypothyroidism rarely causes just one symptom. The symptoms often come on gradually and can therefore be normalized by the person affected. A person may think that the weight gain is due to less exercise, that the constipation is due to the diet and that the depression is related to work.
Common signs are:

- dry skin and dry hair
- constipation
- slower thinking or poorer memory
- depressed mood
- hoarse voice
- slight weight gain
- a feeling of swelling, especially in the face

Not everyone gets all the symptoms. It is also possible to have clear symptoms even though the changes in the blood tests are still relatively small. That is why it is not enough to guess the cause based on the symptoms alone, but the diagnosis needs to be confirmed with blood tests.
The most common causes of the thyroid gland producing too little hormone
The most common cause of hypothyroidism in areas where iodine deficiency is not a major problem is Hashimoto's thyroiditis. It is an autoimmune disease in which the immune system attacks the thyroid tissue. The damage often develops slowly over several years, and the body has time to adapt before the symptoms become clear.
Hypothyroidism can also occur after treatment for other thyroid diseases, such as surgery or radioiodine treatment. Certain medications can contribute, and more rarely the condition is due to disease in the pituitary gland, the gland in the brain that controls the thyroid gland with the hormone TSH.
Some women see temporary or permanent effects after pregnancy. There are also people who have elevated TSH but still normal free T4, what is called subclinical hypothyroidism. In this case, the symptoms may be few or nonspecific, but the test results may still need to be followed over time depending on the situation, symptoms and other findings.
How to make the diagnosis with blood tests
When hypothyroidism is suspected, TSH is the most important first blood test. TSH is formed in the pituitary gland and acts as a control signal to the thyroid gland. If the thyroid gland produces too little hormone, the body tries to compensate by raising TSH.
To understand the whole picture, free T4 is also usually analyzed, i.e. the part of the thyroxine that is available to the body's tissues. The combination of high TSH and low free T4 indicates clear, so-called manifest hypothyroidism. If TSH is elevated but free T4 is still normal, it may be subclinical hypothyroidism.
In some cases, antibodies against thyroperoxidase, often called TPO antibodies, are supplemented. They can support that the cause is Hashimoto's thyroiditis. T3, however, is rarely used to diagnose hypothyroidism, since this test often remains normal for longer and therefore provides less guidance on this particular issue.

In practice, thyroid tests is often assessed together with other common causes of fatigue, such as anemia, iron deficiency, B12 deficiency, inflammation or disorders of blood sugar balance. Treatment for hypothyroidism and why the right dose matters
The standard treatment for hypothyroidism is levothyroxine, a drug that replaces the body's lack of T4. The goal is not to provide “extra energy”, but to restore a normal hormonal balance so that the body’s functions return to their normal level. When the dose is correct, symptoms often improve gradually, but it can take weeks before the effect is fully noticeable.
The treatment is followed up with new tests, usually after 6 to 8 weeks when the dose has been started or changed. This is because TSH reacts slowly. Only when the values have stabilized can it be determined whether the dose is adequate.
A recurring question is why fatigue persists despite the initiation of drug treatment. This may be because the dose is not yet optimally adjusted, that the medication is taken in a way that reduces absorption in the body, or that fatigue is due to several interacting factors. For example, calcium and iron supplements can impair the absorption of levothyroxine if they are taken too close to each other.
Another important question is whether you can treat yourself with dietary supplements or “natural” thyroid preparations. If hypothyroidism is diagnosed, blood tests and medical follow-up will guide treatment. Too high a dose can cause palpitations, internal stress, difficulty sleeping and, in the long term, increase the risk of atrial fibrillation and osteoporosis, among other things.
When should you get tested for hypothyroidism?
There are reasons to consider testing if you have new or prolonged fatigue without a clear explanation, especially if it is combined with chills, constipation, dry skin or unexplained weight gain. The same applies if you have a hereditary predisposition to thyroid disease or another autoimmune disease, such as type 1 diabetes or celiac disease.
Seek medical attention urgently if the fatigue is pronounced and at the same time comes with clear general effects, confusion, pronounced chills, swelling or a noticeably slow pulse. It is uncommon, but severe untreated hypothyroidism can become a serious condition.
When fatigue does not go away despite vacation, better sleep or changed routines, it is often more valuable to measure than to continue guessing. A blood test cannot explain everything, but it can provide a clear next step and distinguish between things that need treatment and things that require other investigation.
