Blood tests cannot measure hair loss per se, but they are a valuable tool for detecting underlying causes such as iron deficiency, thyroid disorders and certain nutritional deficiencies (especially in diffuse hair loss). In the case of genetic or patchy hair loss, the value of the tests is more limited as the diagnosis is primarily made clinically. If the tests show imbalances, it often takes a few months after starting treatment for the hair loss to subside, as the growth cycle of the hair follicles is slow.
Many people first notice hair loss in the shower, on the pillow, or when their legs suddenly look wider in pictures. For some, it comes after an infection, pregnancy, or a period of rapid weight loss. For others, it creeps up on them without a clear explanation, and that's when the question often becomes the same: can blood tests show why their hair is thinning?
Hair loss - what can blood tests show, and when do they really help?
Hair loss is a symptom, not a diagnosis. Blood tests cannot therefore "measure hair loss" per se, but they can find medical conditions that contribute to increased hair loss, especially in diffuse hair loss where hair falls from the entire scalp rather than in clear patches. Common examples are iron deficiency, anemia, thyroid disorder, and sometimes vitamin B12 deficiency or folate.
At the same time, there are many forms of hair loss where blood tests have limited value. Hereditary hair loss in men and women is primarily a clinical diagnosis, i.e. something that is assessed based on the pattern of the scalp, medical history and sometimes dermatoscopy or trichoscopy. Blood tests are then mainly used when the symptoms suggest that something more than genetic susceptibility may be at play.
What causes of hair loss can blood tests reveal?
The most common medical cause that blood tests can detect is iron deficiency. Iron is needed for many of the body's cells, and low iron stores can cause increased hair loss even before you develop obvious anemia. Ferritin is the blood test that best reflects the body's iron stores, but the interpretation must be made in context because ferritin can also rise in inflammation, liver damage and certain chronic diseases.
Thyroid gland is another important piece of the puzzle. Both hypothyroidism and hyperthyroidism can disrupt the hair follicle growth cycle and trigger increased hair loss, often in the form of telogen effluvium, i.e. increased shedding a few months after the body has been exposed to a stress. Therefore, TSH is often included in the investigation, and sometimes also free T4 depending on test results and symptoms.
Blood tests can also show vitamin and nutritional deficiencies. vitamin B12, folate, Vitamin D and zinc are discussed in particular in diffuse hair loss. Here the evidence is more mixed than for iron and thyroid disorders, but several studies and reviews show that low levels of ferritin and vitamin D are more common in telogen effluvium, while zinc and B12 may be relevant in selected cases, for example in case of a one-sided diet, malabsorption, after bariatric surgery or clear deficiency symptoms.
In women with hair loss together with irregular menstruation, infertility, acne, increased body hair or other signs of hormonal influence, blood tests can also be directed at androgen excess. In this case, testosterone, DHE and sometimes prolactin may be relevant, especially if PCOS or another endocrine disorder is suspected. It is not about everyone with female pattern hair loss needing extensive hormone testing, but about choosing tests when symptoms point in that direction.
What blood tests are usually linked to hair loss?
Which tests are reasonable depends on what the hair loss looks like. In diffuse hair loss, a package of blood status and ferritin as well as thyroid tests is often recommended, since these findings are both common and treatable. If the medical history suggests nutritional deficiency or other systemic disease, the investigation can be broadened.
Common blood tests for hair loss are:
- blood status, to detect anemia and sometimes indirect signs of deficiency
- ferritin, sometimes together with iron status such as transferrin saturation
- TSH and if necessary free T4
- vitamin B12 and folat
- vitamin D
- zinc in selected cases
- CRP or another inflammation marker when you need to interpret ferritin more confidently
- testosterone, DHE and prolactin when hormonal disorders are suspected in women
A common question is whether “all vitamins” should be analyzed directly. This is rarely necessary. A more accurate strategy is to start with the tests that have the best medical relevance based on symptoms, diet, medications, menstrual patterns, weight history and whether the hair loss is increased shedding or gradual thinning.
It is also worth knowing that normal blood tests do not rule out a hair loss disease. Hereditary hair loss, alopecia areata and drug-induced hair loss can be present despite normal laboratory values. Blood tests are therefore a complement to the clinical assessment, not a replacement for it.
When are blood tests particularly justified in hair loss?
Blood tests are most justified when the hair loss is diffuse, new or combined with other symptoms. Examples include fatigue, shortness of breath, heavy menstruation, rapid weight loss, feverish periods, gastrointestinal problems, feeling cold, palpitations or changes in the menstrual cycle. This increases the likelihood that there is a treatable underlying cause.
This also applies when hair loss has started 2–4 months after a clear stressor. Telogen effluvium often occurs delayed after, for example, COVID infection, other feverish illness, surgery, childbirth, severe psychological stress or rapid dieting. In this situation, blood tests can help distinguish between a transient reaction and a simultaneous deficiency state that slows recovery.
In the case of patchy hair loss, such as in alopecia areata, the picture is different. The diagnosis itself is usually made clinically, but since alopecia areata is linked to autoimmunity and a higher incidence of thyroid disease, targeted testing may be relevant, especially in cases of symptoms or heredity for autoimmune disease.
Treatment depends on the cause – not just the hair loss itself
If blood tests show iron deficiency, the cause of the deficiency is treated first, for example heavy menstruation, insufficient intake, blood donation or gastrointestinal losses, while iron therapy is often given. If a thyroid disorder is found, treatment is directed at the hormonal imbalance. When the deficiency or hormonal disorder is corrected, hair loss usually decreases, but this rarely happens immediately because the hair follicle cycle is slow.
With telogen effluvium, time is part of the treatment. Once the triggering factor has disappeared, the loss usually decreases within months, and visible regrowth comes later. It is common for hair loss to continue for a period of time even though the test results have already improved, which may seem contradictory but reflects how the hair growth cycle works.
In female or male hereditary hair loss, the first-line treatment is often minoxidil. In some women with hormonal influences, antiandrogen treatment may be appropriate after medical assessment. If female hair loss occurs together with acne, increased hair growth or menstrual disorders, it is not necessary to be satisfied with stating "hereditary hair loss", but can also benefit from investigating hormonal causes.
In alopecia areata, other treatments are used, since the disease is autoimmune. Small patches are often treated locally, while more severe cases can be treated with immunomodulatory drugs. There are now approved JAK inhibitors for alopecia areata in certain situations, but they require specialist assessment and careful follow-up.
Signs that it is time to check your hair loss
You should react if your hair loss comes on quickly, if you also feel tired and lack energy, if you have heavy periods, if you have recently lost a lot of weight or if you are losing hair from your eyebrows or other parts of your body. The same applies to itching, redness, scarring or clear bald patches, as such findings may indicate other diagnoses than normal diffuse hair loss.
If you see more hair than usual but your scalp looks normal, testing can be a good first step. A well-thought-out test package can provide answers to whether the body is signaling iron deficiency, anemia, thyroid disorder or nutritional deficiency. If normal values are obtained, the next step is often to assess the pattern, time course and possible triggering factors of the hair loss more carefully.
