Blood analysis of Methoxycatecholamines

Methoxycatecholamines, fP is a specific blood test used to analyze the body's metabolism of stress hormones known as catecholamines. The analysis measures stable breakdown products of adrenaline, noradrenaline and dopamine in free plasma and thus provides a picture of long-term catecholamine production.

The analysis includes the following biomarkers:

  • Methoxyadrenaline (metabolite of adrenaline).
  • Methoxynoradrenaline (metabolite of noradrenaline).
  • Methoxytyramine (metabolite of dopamine).

Since the stress hormones themselves are released pulsatilely and vary greatly over time, their metabolites are more stable and therefore more suitable for diagnostics. Methoxycatecholamines in plasma are today considered one of the most sensitive tests for investigating overproduction of catecholamines.

The role of methoxycatecholamines in the body

Catecholamines are produced primarily in the adrenal medulla and in the autonomic nervous system. They have an impact on the body's stress response and affect, among other things, blood pressure, heart rate, vascular tone, blood sugar and nervous system activity.

Once catecholamines have been released and exerted their biological effect, they are rapidly broken down into methoxylated metabolites via the enzyme catechol-O-methyltransferase. These metabolites, methoxycatecholamines, lack their own hormonal effect but serve as important biomarkers for how active the stress hormone system is over time.

Why is methoxycatecholamine analysis performed?

Analysis of methoxycatecholamines is primarily used in cases of suspicion of hormone-producing tumors that cause overproduction of catecholamines, primarily pheochromocytoma and paraganglioma. These conditions are relatively uncommon but can cause serious symptoms and complications if not detected. The analysis may therefore be relevant in the event of symptoms or clinical findings that indicate uncontrolled stress hormone production, especially when the symptoms are recurrent or difficult to explain.

Reasons for analysis of methoxycatecholamines include:

  • unexplained or difficult-to-treat high blood pressure.
  • recurrent attacks with palpitations, sweating and headaches.
  • suspected pheochromocytoma or paraganglioma.
  • stress hormone symptoms that cannot be explained by other conditions.
  • heredity for hormone-producing tumors or genetic syndromes (e.g. MEN2, VHL, SDHx).

Viktigt att tänka på inför provtagning

Metoxikatekolaminer analyseras genom ett blodprov i fritt plasma. För att minimera risken för falskt förhöjda värden är det viktigt att provtagningen sker under lugna förhållanden. Stress, kroppsläge, fysisk aktivitet, koffein, nikotin och vissa läkemedel kan påverka katekolaminomsättningen. Provtagning rekommenderas efter vila.

Vad visar resultatet?

  • Normala nivåer: Talar emot feokromocytom eller paragangliom.
  • Förhöjda nivåer: Kan indikera överproduktion av katekolaminer och kräver därav vidare medicinsk bedömning och kompletterande utredning.

Resultaten tolkas i relation till symtom, klinisk bild och övriga laboratorieprover. Ett avvikande värde innebär inte automatiskt diagnos, men är en viktig signal som kan leda till vidare utredning.
