Before the sampling

  • Medicines and dietary supplements: Avoid intake of any iron tablets on the same day as the test if your test contains the marker iron. Postpone the intake of any thyroid medication until after the sample is taken and avoid dietary supplements or drugs with a high content of biotin before tests relating to thyroid markers.
  • Rest for 15 minutes in place on the sampling unit.
  • Bring valid identification.

T4, or thyroxine, is a hormone produced by the thyroid gland and is essential for the body's metabolism. T4 acts as a precursor to the more active hormone T3, and is converted to T3 in the body's tissues. T4 affects energy metabolism, body temperature, heart function, and mental health.

Normal levels of free T4 are usually between 12–22 pmol/L, but reference values ​​may vary slightly between laboratories. Total T4 has different reference values ​​and is less commonly used in routine diagnostics.

You should test T4 if you have symptoms of a thyroid disorder such as fatigue, weight changes, feeling cold, or palpitations. T4 is tested along with TSH to give a complete picture of thyroid function.

An elevated free T4 level may indicate hyperthyroidism (overactive thyroid gland). Common causes include Graves' disease or thyroid hormone overdose. Symptoms include weight loss, sweating, anxiety, and palpitations.

Low free T4 is usually indicative of hypothyroidism (underactive thyroid), which can cause fatigue, weight gain, chills, dry skin, and depression. It may be due to autoimmune disease (Hashimoto's) or lack of hormone production after treatment for hyperthyroidism.

Levothyroxine, often sold under the brand name Levaxin, is a synthetic hormone that is equivalent to the body's natural T4 (thyroxine). In hypothyroidism (underactive thyroid), Levothyroxine is used to replace the deficient hormone production. When you take Levothyroxine, the levels of free T4 in the blood increase, which helps to normalize the body's metabolism and relieve symptoms such as fatigue, chills and weight gain.

Treatment is adjusted individually and often starts with a low dose that is gradually adjusted based on blood tests for TSH and free T4. The goal is to achieve a hormonal balance where TSH is within the reference range and symptoms decrease. It usually takes several weeks before the effect of the treatment is fully noticeable, since thyroid hormones act slowly in the body. Regular follow-ups are important to ensure the correct dose and avoid over- or undertreatment.
