DEXA is the most accurate medical method for measuring fat and muscle at a single point in time, while the faster gym method InBody is sensitive to the body's fluid levels but works great for tracking your progress over time if the measurement conditions are identical.
Two people with exactly the same weight can have completely different ratios of fat, muscle and abdominal fat. This becomes especially clear when results fail to show, waist circumference increases despite the scale standing still, or when weight loss causes you to lose strength instead of fat. To understand what is actually happening in the body, more than a regular scale is required — with InBody and DEXA being two popular methods.
What do DEXA and InBody actually measure?
Both DEXA and InBody are used to estimate body composition, i.e. how the body is distributed between fat mass, fat-free mass and in some cases more detailed sub-values such as regional fat distribution or visceral fat.
DEXA, also written DXA, is an X-ray-based method where the body is scanned with two different energy levels of X-ray radiation. The method has long been used in healthcare for bone density, but can also measure total fat mass, fat-free mass and how tissue is distributed across the body. DEXA is described as an established method for body composition, especially in clinical follow-up and research.
InBody is instead based on bioelectrical impedance, often abbreviated BIA. A very weak electrical current passes through the body, and the device calculates body composition based on how easily the current passes through different tissues. Since water conducts electricity better than fat, the result depends on the body's fluid distribution, body position and how the measurement is made.
For someone standing in a gym or at a reception, the difference feels clear. DEXA is a medical examination on a bed with imaging equipment, while InBody is a quick standing test that often takes less than a minute.
DEXA or InBody – which method is more accurate?
When it comes to which method gives the most accurate picture at a single time, DEXA is usually stronger. Reviews describe DEXA as one of the most accurate methods of body composition in clinical studies, while multi-frequency BIA is more often seen as a more practical alternative in everyday life.
This does not mean that InBody is “wrong”, but that precision and accuracy are not the same thing. A BIA device can be consistent enough to track change over time, but still show different absolute numbers than DEXA. This is clinically relevant. A person can be told 29 percent body fat in one device and 33 percent in another without the body actually having changed. Therefore, figures from different methods should not be mixed together when following development over time.
The fairest conclusion is therefore that DEXA usually gives the best picture of the current situation, while InBody is often sufficient for follow-up if the same device and the same test conditions are used each time.
When does InBody become misleading?
The biggest weakness with InBody and other BIA is the sensitivity to fluid. If you drink a lot of water just before the measurement, exercise hard, test yourself late in the day or have urine left in the bladder, the result can be affected.
A concrete example is the person who measures himself after a lunch session at the gym and thinks that his fat percentage has suddenly risen or his muscle mass has changed unexpectedly. It does not have to reflect real changes in fat or muscle, but may be due to the body having a different fluid distribution at that time.
This is especially true in situations where fluid balance often varies:
- after exercise or sauna
- with menstrual cycle-related fluid retention
- with high or low salt load
- with diabetes, kidney disease or edema
- with rapid weight loss or severe obesity
For those who use InBody, standardization is therefore more important than many people think. The same time of day, fasting or under similar meal conditions, before exercise and after a visit to the toilet provides more useful serial measurements. InBody's own manuals recommend morning measurements, on an empty stomach, before exercise and under similar conditions every time.
When is DEXA best suited – and when is InBody enough?
DEXA is best suited when you need a more reliable basis for decision-making. This can be in the case of major weight loss, after obesity treatment, when suspected low muscle mass, when following up on sarcopenia or when the fat distribution in the body is medically relevant. DEXA can also be more informative when the scale creates confusion. A person who starts strength training can have a stable weight but at the same time reduce fat mass and increase muscle mass. In this case, DEXA often gives a clearer picture of what is actually happening in the body.
However, InBody often goes a long way in practice when the aim is to follow trends over time. For many, speed, availability and low cost are major advantages. The decisive factor is therefore not only which method is "best" in theory, but which question you want to get an answer to. If you want the highest possible accuracy at the moment, DEXA is the way to go. If you want to track the direction of your change month by month, InBody may be more than enough.
How to interpret the results without drawing the wrong conclusions?
The most common mistake is to get stuck on a single number. Body fat percentage, muscle mass and visceral fat are useful measures, but they need to be put in context with waist circumference, blood pressure, blood sugar, blood lipids, physical function and how the person feels. Another practical issue is radiation. DEXA uses ionizing radiation, and although the dose is low, this should be taken into account. Pregnant women should not have DEXA for body composition.
Body composition is most valuable when used as a starting value. It can help you detect hidden risk, monitor the effect of exercise or weight loss and see whether the body is losing fat, muscle or both. That difference is rarely visible on the scale alone.
