What are normal testosterone levels?
According to the reference intervals used in healthcare in Sweden, the testosterone level in the blood of a healthy individual should normally be between approximately 12 to 35 nmol/l. The interval is relatively wide, but the average value for men who have tested themselves with us is 17.37 nmol/l. Note that testosterone levels can vary up to thirty percent over the course of a day, with higher levels usually measured in the morning. These variations are most noticeable in younger men and tend to even out with age. Since the reference values for testosterone are based on morning values, it is important to take the blood sample in the morning if you want to measure your testosterone.
It is also important to understand that the normal level of testosterone can vary considerably between healthy individuals of the same age. The level that is normal for one person may not be normal for another. In medical assessment, the evaluation is therefore mainly based on symptoms. If symptoms of testosterone deficiency and a simultaneous low testosterone level are present, further investigation and possible treatment may be considered.
Common symptoms of testosterone deficiency
Signs of low testosterone include decreased sex drive, fatigue, decreased muscle mass, increased fat accumulation and impaired physical performance. Many men also experience mood changes, such as irritability, depression or decreased motivation, which can affect both work life and relationships. In the long term, low levels of testosterone can lead to reduced bone density and an increased risk of osteoporosis. In addition, symptoms such as sleep disorders, reduced body hair and impaired cognitive ability may also occur. However, it is important to note that these symptoms vary from person to person and that a combination of symptoms together with a low testosterone level often forms the basis for medical assessment and possible treatment.
Testosterone levels in men begin to naturally decrease from around the age of 30. This decline is gradual and is estimated to be approximately 1–2% per year. This means that a man at the age of 50 may have lost up to 20-40% of his original testosterone levels. The gradual decline is a normal part of aging, but for some, levels can drop more quickly, which can cause symptoms of testosterone deficiency even earlier in life. Therefore, it is important to follow up on symptoms in combination with testosterone measurement to determine whether treatment is necessary.
Interpretation of S-testosterone
In men, testosterone levels drop by about 30% one to three hours after a morning meal, making fasting testing important for accurate results. Testosterone varies greatly throughout the day, with highest levels in the morning and lowest around midnight, and a short half-life in the blood of about four minutes. For women, the phase of the menstrual cycle affects the assessment of testosterone levels, but sampling time and fasting have no clinically significant effect. However, standardized sampling time is important for comparability.
