Tuberculosis Ag 2, TB2, is part of the IGRA assay and measures interferon-gamma responses from both CD4+ and CD8+ T cells after exposure to tuberculosis-specific antigens. TB2 is always assayed together with TB1.

Tuberculosis Ag 2 (TB2) is the second antigen component of the IGRA assay for tuberculosis. The marker measures how the immune system's T cells respond to specific antigens from bacteria within the Mycobacterium tuberculosis complex. TB2 is reported as a quantitative IFN-gamma response in IU/mL and is always analyzed together with Tuberculosis Ag 1, TB1.

TB2 captures a broader T-cell response

What is particularly characteristic of TB2 is that the antigen stimulation is designed to activate both CD4+ T helper cells and CD8+ T cells. This distinguishes TB2 from TB1, which primarily reflects responses from CD4+ T cells. By also including the responses of CD8+ T cells, TB2 can provide information about a broader part of the cell-mediated immune response to tuberculosis antigens.

CD8+ T cells contribute to the immune response to tuberculosis

CD8+ T cells play an important role in the body's defense against intracellular infections. They can recognize infected cells, release interferon-gamma, and help direct the immune response to microorganisms that are inside the body's cells. When stimulated with TB2, both CD4+ and CD8+ T cells can contribute to the amount of interferon-gamma measured in the laboratory.

TB2 measures the release of interferon-gamma

When T cells recognize the tuberculosis-specific antigens used in the assay, they can be activated and release interferon-gamma, IFN-γ. The amount of interferon-gamma is then measured and reported in IU/mL. The TB2 value thus shows the degree of immunological reaction after antigen stimulation, but the value should not be interpreted as a standalone diagnostic result.

TB2 is assessed together with TB1

Tuberculosis Ag 2 is always analyzed together with . The two markers measure similar immunological reactions but from partly different T-cell populations.

TB1 primarily reflects the response of CD4+ T cells, while TB2 includes responses from both CD4+ and CD8+ T cells. The two results therefore complement each other in the overall laboratory assessment.

The difference between TB2 and TB1

The difference between the markers lies primarily in which T cells are stimulated. TB2 is designed to capture a broader cell-mediated immune response, while TB1 is more focused on the response of CD4+ T helper cells.

Although TB1 and TB2 may yield different quantitative values ​​in IU/mL, the difference between these values ​​has no established independent diagnostic significance. The results should therefore be interpreted together and in their clinical context.

TB2 indicates immunological response, not active tuberculosis

A clear TB2 response shows that T cells are reacting to the tuberculosis-specific antigens included in the assay. However, it does not in itself indicate that the person has active tuberculosis disease.

TB2 should therefore be seen as a measure of the immune system's reactivity to tuberculosis antigens and not as an independent marker of active infection or disease activity.
