Maximizing your training is not just about more sessions or harder loads. For performance, recovery and long-term health, several blood values play a big role, especially if you train a lot, run long distances, strength train regularly or want to reduce the risk of overload. The right tests can help you detect iron deficiency, energy deficiency, vitamin deficiency or medical conditions that cause the body to not respond as it usually does.
Iron values and blood status – when oxygen transport becomes the bottleneck
If you become unusually tired during workouts that previously felt easy, it is reasonable to start with blood status and iron-related tests. Hemoglobin shows the blood's ability to transport oxygen, while ferritin reflects the body's iron stores. Low levels can cause decreased energy, shortness of breath, palpitations, poorer recovery and sometimes headaches or restless legs.
This is particularly relevant for endurance athletes, people who sweat a lot, menstruate, eat little meat or train with a high total volume. Iron deficiency can be present even without obvious anemia, and then it is often first noticed as impaired performance rather than as a feeling of illness. At the same time, low ferritin does not always mean poorer performance if hemoglobin is normal, so the finding needs to be interpreted in context and not treated casually.
For those who want to understand their results, the combination is more important than a single value. A blood test together with ferritin, sometimes also transferrin saturation and CRP, gives a more reliable picture than ferritin alone because ferritin is also affected by inflammation.
Vitamin D and B12 – small deficiencies that can be noticed in major training goals
D-vitamin is best known for the skeleton, but the levels also have an impact on muscle function, immune defense and the risk of stress fractures. In people who exercise a lot indoors, live in the dark during the winter months or rarely spend time in the sun, low levels can be a partial explanation for recurring infections, diffuse muscle fatigue or slow recovery. Research shows that vitamin D is important for health, but the direct performance gains from supplementation seem to be mainly relevant when there is actually a deficiency to begin with.
Vitamin B12 is another value that can affect both stamina and nerve function. Deficiency can cause fatigue, reduced concentration, tingling, numbness and in some cases anemia. It is especially relevant for vegans, the elderly, people with gastrointestinal disease or those who use certain medications for a long time. If you experience that your body feels “discharged” even though your training is otherwise reasonably planned, B12 is one of the tests that may be worth checking.
Thyroid values and energy deficiency – when the problem is not in motivation
It is easy to interpret poor performance as poor discipline, but sometimes it is about physiology. Thyroid gland regulates metabolism, and disorders in TSH and free T4 can cause fatigue, chills, low pulse, stiffness, a feeling of weakness or unexpected weight change. In those who exercise regularly, it can be mistaken for overtraining or insufficient recovery.
Another important explanation is low energy availability, which in sports medicine is described as relative energy deficiency. It occurs when energy intake over a long period of time is not sufficient for both exercise and the body's basic functions. The consequences can be impaired performance, increased risk of injury, missed periods or menstrual disorders, low libido, depression and slow recovery. Both women and men can be affected.
There is no single blood test that makes the diagnosis, but laboratory values can provide important clues. Declining ferritin, hemoglobin, vitamin D and sometimes changes in hormone tests can fit the picture, especially together with symptoms and exercise history. If you train a lot but at the same time try to be in a calorie deficit to become lighter, faster or “more defined”, it is wise to review both nutritional intake and test results before your body gives you a clearer answer.
Muscle breakdown, inflammation and recovery – values that must be interpreted correctly
Many people who train hard want to check creatine kinase, CK, to see if recovery is sufficient. CK is an enzyme that rises when muscles are loaded and can be clearly elevated after heavy strength training, intervals, races or unusual eccentric moments. At the same time, CK is an absolutely crucial marker when suspecting more serious muscle damage, such as rhabdomyolys (acute, severe muscle breakdown) or other underlying muscle diseases. This means that a high value is not automatically dangerous after training, but that it must be interpreted in its proper context.
That is precisely why CK is difficult to use as a sole compass. Two people can have completely different levels after similar training, and the same person can vary greatly between different weeks. If the test result is to provide a reliable picture, it should be interpreted together with your training load, your symptoms and other values.
Similar reasoning applies to inflammation markers such as CRP. A slightly elevated CRP can be due to infection, inflammation or in some cases affected by heavy physical exertion. Therefore, these tests are most useful when interpreted together with symptoms, other values and your training load, not as isolated “green or red” signals.
Blood lipids, glucose and long-term health – values that affect more than the year's peak form
Those who train a lot may still have risk factors for cardiovascular disease. Training is highly protective, but it does not completely negate the importance of heredity, blood lipids, blood sugar or abdominal obesity. Therefore, it is wise to focus not only on performance markers but also on values that say something about future health.
Here, blood fats are central: total cholesterol, LDL, HDL and triglycerides. For some, Apo B can also provide a better picture of the number of atherogenic lipoproteins, i.e. the particles that contribute to arteriosclerosis. If you do a lot of strength training, bulking, use performance-enhancing drugs or have a strong heredity for early cardiovascular disease, this part becomes especially relevant.
Glucose and long-term blood sugar can also provide valuable information. A weak energy intake, a lot of energy drinks, lack of sleep or weight gain between training periods can affect blood sugar regulation even in people who look well-trained.
When choosing which tests are relevant, you should start from the goal of the training and how the body actually feels. Those who are aiming for a marathon often benefit more from understanding iron status and energy availability than from chasing temporary CK peaks. Those who want to build sustainable capacity over many years also benefit from not missing blood lipids, blood sugar and other values that affect health beyond the next race.
